Q: Does this make changing the bed sheet more difficult?

It does not affect making the bed. They go under the matress and on top of the box spring. So it the same as tucking the sheets without the bed rails.

Q: What is width of rail from right to left?

The width is quite wide, about 18 inches, wider than other bedside rails, and it is large, very firm, And the height can be adjusted，very comfortable to use, and easy to install

Q: How is strap supposed to be attached?

Once You have it positioned, the strap goes from the rail frame to the bed frame to keep it stable/strong. It can all be done from the other side of the bed easily.

Q: Does this have a vinyl plastic smell straight out of the box?

Neither myself or my husband (who regularly uses this bed rail) noticed it having a vinyl plastic smell when we first got it nor since then. I should also tell you that if it did have a vinyl plastic smell, he would have noticed it, as he has a very sensitive sense of smell.
