It is always on a spray setting (low .5gpm, medium 1 gpm, or high 1.5 gpm). This one does not switch to an "aerated" flow. We've found that this one seems to do a much better job than the aerated ones that we had previously. Everyone that tries it wants to get them for their homes. We keep it on low most of the time... especially with a 2yr old and a 4 yr old (they like to let it run a while).

Is there a way to also stop/pause water flow with this device?

No, not on the device itself. You have to have a separate control.

How do you use the plastic key to make it work for external threading? The instructions on the little bag are useless.

If i remember correctly.. there should be a couple of notches on the threaded area. the key should fit right into those notches and just unscrew.

Is this bpa and lead free?

No. The unit I purchased this month has a sticker on the packaging stating it may expose lead. I went to their website. Apparently they sell a lead free version which coats the chrome with ceramic.
