Some of the examples may be a little dated (i.e., references to overhead slides and mailing surveys?), but the foundations of why and how to complete a thorough needs assessment have not changed.  The authors present a very detailed model for novices and experienced practitioners alike, that can be easily modified to fit the needs of your project.  The model takes the reader through three key phases and 14 steps for practitioners to ensure that they have documented, prioritized, and are prepared to analyze the needs of their clients.  This book is still used in many graduate school programs, which speaks to its longevity and validity of the approach, and the cost as far as textbooks go isn't too bad.

Sound explanation of everything one would need to know in order to travel through a Needs Assessment. Highly recommend.
