Layout, spacing and size of font did not align with expectations
I'm not sure if its user error, the form or the person who printed the sign but, the spacing and size of font were not what was expected to the what my interpretation of the form represented.Update - received new sign without notice with a different layout. This layout make more sense and takes advantage of the space. Kudos for making it right. Thank you, upping the stars.

Too small
It’s tiny not at all what I expected

On the thin side
This will require backing with another piece of metal. It flexes too much due to the gauge of metal. It should be twice as thick if you're going to mount it on a post for example.
