Is the top shelf one piece? Can it be lowered or it has to stay across the very top? Is the lowest shelf one piece and can it be hire off the ground?

Don’t know. Box size looked like it might be two pieces, not sure. My husband and daughter assembled this in a closet and noted this item gets stronger as it gets assembled. Both are really impressed with it though.

Can I add wheels to this?

I would not add wheels to this. It would likely tip over when you tried to move it once it was full of clothes.
