Q: How can this be heavy duty when it is only 16 gauge? Can anyone who bought this cord define "heavy duty"?

House wiring is usually 12 gauge or 14 gauge. Ordinary extension cords would be 16 or 18 gauge and not be double insulated. An obvious exception would be short extension cords for a clothes dryer or other appliances that draw a lot of current. Another point about the GE cord is that it has three outlets which is not obvious in the picture. I needed a cord with only one so it wold fit where I had to use it.
