This chart worked very well for my purposes
I sent it to the care facility in a different state where my mom is in hospice. She was having difficulty reading and is unable to go to an eye appointment.I sent this chart so she could hold it like a book and the section she could read clearly was the prescription strength she needed. I sent her readers of that strength and they are working out well for her.Perfect solution.
