Q: Can you get this with 3 pens instead of 2 pens and a pencil? or change the pencil out?

It is actually possible if you take the entire thing apart and have the part that holds the pen or pencil refills. I took 3 of mine apart and made 2 red-blue-plack pens and a triple pencil that I use for drawing with different types of leads.

This pen is rather slim - approx. 7mm in width/diameter

Q: How do you switch from black to red to pencil? Must you interchange the cartages?

No changing. All pens/pencil are inside. When you twist the shaft they rotate out one at a time. stop twisting when you get the color you desire.
