Cant answer that since I don't know what a 400lbs man looks like. I don't think a 400lbs man could even fit one around him not alone what to so with the excess. Can you please clarify your question?

There is a strap on the side for a drink holder

It's a big bag, will be hard to fit in the small locker.

Q: big enough to use for a middle schooler as a book bag?

It depends on your middle schooler. If he or she is used to carrying around all their text books, pens, papers, notebooks, computer device etc. at all times on their person probably not. Good for a lot of storage with a locker dump during the day tho. Hope this helps.
