Q: How noisy is it?? I have a 3 band EQ like this and it gives off tons of electrical noise/interference aka pops, hisses & crackling.

It sounds really good. I hear bench conferences when in court really well. My judge and attorneys whispers during the bench conferences. No cracking or hissing noises.

Q: I need this to boost volume when watching TV with headphones. Does it have an input jack or just output?

Over a dozen years ago, I was hard-of-hearing. I bought the amplifier to improve tv viewing without disturbing my neighbors. It didn't help much. Now, I have serious hearing problems and I thought perhaps the amplifier would be of help. It doesn't help. As a 95 year old the problem is too advanced. I am sorry my experience with the amplifier is of no help to you.

Q: Does it have two microphones, for true stereo out to the headphones? I need something for going to the movies.

I can’t answer about if it has two separate microphones but I can tell you that if you have stereo headphones and you plug them in you will get sound in both ears. So yes to stereo sound.
