Lovely pattern, easy to follow, now get some wool then I'm ready to start knitting.

Confused
I'm an expert knitter even corrected a few Patten mistakes in a harry potter knitting book but the round neck cardigan on this has me stumped. I don't understand the arms hole as daft as it sounds. I don't know at what point on the 3-6 month size I'm supposed to start them. I did it so the pattern on the fourth diamond at the top is Dec but now the arm hole seems massive. Instead of it been in cm from bottom to when the arm hole is supposed to start it just says 19 rows more. Well I did that the first time didn't look right so tried 2 more different ways and it's still not working out. Please help
