How long do these stay cool once they've come out of the freezer?

On skin, alternating with a hot pack, about 20 minutes probably more. Certainly long enough for my purposes.

Are these any good for use on the lower back?

I got mine for my knee and providing I hold them in position with tea towels they are excellent, regarding the lower back again it would be holding them in position but sat in a chair with them in position it should not be a problem. I have only used mine from the freezer I have not tried them heated but if they as good heated as they are cold and for the price go for it!

Nope. I use a cotton cloth to wrap around it ... stops it being too hot or too cold on the skin directly.
