Where is this corkscrew manufactured?

I believe it is manufactured in China. However the quality is much higher than what I have seen with clones.

Is is possible to repair the spiral part of the cork screw when it breaks, as mine has?

I don’t see how it could be repaired. The spiral motion is such a critical part of the opening process. I don’t think a repair would hold

It is used like any other corkscrew. First, cut off the foil top of the wine bottle. Second, position the black plastic on either side of the bottle neck, so the notch about an inch from the bottom rests comfortably on the lip of the bottle. Third, begin the process of screwing the corkscrew into the cork. You should see the cork lifting up from the bottle neck. Continue this process until the cork emerges from the bottle.
