What exactly are the dimensions? The description says 4". Also says 12" hook, 12"loop. But dimensions say 4.5 x 4.5 x 0.8?

This is a 12" long by 4" wide piece of each side - hook side and the loop side also. The 4.5x4.5x.08 dimensions listed are the package dimensions that Amazon ships.

Do you think fabric glue would hold the loop side to a nylon backpack? If yes, fabric glue recommendations?

Not sure but you could try. Sewing would be best as Velcro sticks to each other vey well and constant pulling may cause it to pull away from glue.

How many pounds can this hold?

Need more input. Are you trying to hold something on a vertical surface? You would need to nail or screw the one side to the surface. It should hold up to 20 pounds, if the part on the surface you are attaching is just as thoroughly attached to the strip. In other words, the strength of the velcro is not in question, the surface you are sticking it to can have many variables, porous or non porous, metal, wood (treated or untreated), plastic, etc. Also consider the temperature where it is attached. You are going to have to experiment to know for sure. Sorry I can't be more help.
