Has IFIT made any attempt to resolve the very poor video quality on the S22i studio bikes? I do not want to subscribe if the video is not HD.

At this point, there is very little indication that HD video streaming will be available any time soon. The parent company, ICON, along with Peleton, is being sued. They now only offer programming in 540P. Which is awful picture quality. Don’t purchase it IMHO.

Does this work with an already created iFit profile or do I have to create a new one?

It can work, but you'll have to call iFit in order to get them to activate the membership and tie it to your iFit account. It's a bit frustrating from my experience.
