Where would I find the model number on my canopy?

There should be a small tag somewhere on the underside of the canopy. If it is gone double check the measurement of your gazebo. If it is 10x12 it should fit. We have one from a different brand, not eurmax, and it fits perfectly. It even came with a tool that makes it very easy to put it on the frame.

Fits extremely well. Easy to install. Be sure to tie off the top vent ties prior to pulling all 4 corners.

Where is the item being shipped from?

I'm not exactly sure, but it IS in the United States. I think it comes from the midwest.
