It works and good quality

I guess we have more vaccines than when I needed it. To list them all out, you'd fill this up quick. There's a 3 line section and then this "other" that leaves you more room. You can add medical contraindications to vaccines, personal health history, regular meds, and glasses information as well. It's a thick paper, not cardstock as it was 20 years ago. Still should fit within a passport, although it comes in a nice plastic holder.

This is the second pamphlet I've used. Learned with the first one how very helpful it is to have all documentation in-hand and on one log. Now I know where to refer people who ask about where they can get one! Thank you for the case as well.

This is a waste of your time and money when you travel is good for nothing

I love this card becard because I can keep track of all my vaccinations in one place. Whenever I receive a vaccination of any sort I have the provider enter it on this card which I keep stapled in my passport.
