Q: Does this have a self-adhesive backing or do you have to drill into the wall to put it up?

I've drilled it.. it's brilliant for kid's..

Q: How long do the 8 batteries last before they need changing?

Believe it or not the sanitiser gel dispenser is still operating with original batteries, its been operating perfectly, I am now very pleased with the product.

Q: How is this attached to the wall, does this need to be drilled? Any instructions?

Yes, via 2 screws (screws and plugs supplied or use your own). Straight forward. I guess you could use adhesive pads if you had some (doesn't come with any) but you would need to ensure they are very strong as the unit is pretty heavy when its full of soap and you have added the battery weight. Personally I wouldn't attach it with anything other than screws.
