Q: Are there ball bearings in the wheels or only in the swivel?

They are sealed so can't be seen. They are likely caged ball bearings inside the seals. My problem is with the swivel bearing. They are ball bearings between two plates and in 4 months I've had one go bad. I'm on a hard tile floor so there is no reason for this to go bad. No idea how to see about a warranty so I guess I'm out so I'll suck it up and pay for a new set. I'll keep the good ones for back up. More to come after 4 months of the next set.

Q: can you get only four?

Our office chair wheels set of 5 can not sell separately, if you need 4, the rest 1 can be saved for the next replacement. Hope this helps you.

Q: What are the wheels made of? I'm always suspicious of a seller who doesn't give critical product information.

It's in the title. TPR is Thermoplastic Rubber. It's basically a combination of plastic and rubber features.

Q: How do these perform on med pile carpet?

I have hardwood floors only. I am not aware of how they work on medium pile carpet. I have a very thin area rug, a ruggable, and they catch a bit. Otherwise, they work well for me.
