Q: How often can/should this be used and how long should you use it at a time?

I used for 10 minutes, twice a day. I felt some relief in 2 days and continued for almost 2 weeks for complete relief. . So far I haven't had to use it again.

Q: How tight should it be pulled? Is it supposed to smoosh my cheeks?

I was so desperate for relief that I was prepared to pull as hard as I could stand but my husband it the voice of reason. He suggested easing into the higher tension. So I used it for 10 minutes twice a day for 2 days with just moderate tension. Then I tightened the tension and yes it smooshes your cheeks but with wonderful results.

Q: Description states calibrated for 0 to 40 pounds. Does that mean it will automatically release tension if more that 40 pounds is applied?

With a design of the spring-loaded meter, you have an better idea of how much tension you are applying to your neck. The scale being from 0-N (0LB) to 200-N (40LB) allows you to know the traction parameter.

Q: Can you set it to a certian poundage

The tension is not set before the traction. The scale will show the pounds as the ratchet cord is pulled - there needs to be tension on the device before the pound scale is visible. You can't see the scale as you use the device - someone else would need to read it for you.
