Q: Is the size chart actually true to the size of the shirts? Or should I size up from what I should wear based off of the chart?

I usually wear a medium so I ordered a medium but it's too big so I would recommend ordering one size smaller than your usual. I ordered a small the second time so we'll see if it fits better.

Q: Does it need to be ironed after washing it?

Just washed it for first time today and it looks like it will need ironing. But the instructions recommended dry cleaning

Q: Is the size accurate?

Yes, but if you want a loose fit, maybe get one size up.
