Q: Do you pay extra for the neck flap, or does it come with the hat ?

There are 2 colors that have the neck flap already attached, you just have to make sure you select the right one. Personally - I would not get this model unless there is some way to store the flap inside the hat. I have a couple of hats (different brand) where you roll the flap into a compartment under the rear brim.

Q: Is the hat machine washable? Getting as a gift and that's a must have.

EHHH, yes and no. I wash mines because I use it as a work hat and I sweat pretty hard in it. I say no because it's surely lost it's shape. It's still usable though.
